age-friendly communities

This functional plan uses the results of a community survey to propose several age-friendly interventions. Each of the WHO’s eight domains of livability includes a chart with activities, responsible agencies, a timeline, resources, barriers, partnerships, metrics, and outcomes. The Appendix includes a copy of the Community Survey used to assess community needs as well as a detailed analysis of the survey results.
