This functional plan integrates the age-friendly and dementia-friendly frameworks to propose a series of recommendations related to health and community support, housing, social participation, transportation, and becoming dementia-friendly. Each section discusses the vision, community feedback, current conditions, and goals and actions, and all recommendations are summarized in the Appendix. It also includes a call to action for municipalities to help implement the county’s plan (p. 35).
